Informed Consent
Informed consent form for the processing of genetic data and the execution of genomic analysis, pursuant to Art. 9(2)(a) of EU Regulation 2016/679 (GDPR).

1. Subject of the test
The Genomatik test is a genomic analysis via microarray genotyping (Illumina GSA) of 612,847 SNPs from a saliva sample.
The report covers: genetic predispositions (27 conditions), CPIC pharmacogenomics (9 genes, 45 drugs), 84 phenotypic traits, carrier status (22 conditions), Wellness Rating.
2. Purpose of the test
Exclusively informational purpose. Genomatik does not provide diagnoses or prescribe therapies. Pharmacogenomic information is based on CPIC guidelines.
All therapeutic decisions must be made by the treating physician.
3. Voluntary nature
Participation is voluntary. The data subject may withdraw consent and request sample destruction at any time.
4. Sample collection
Saliva sample collected at home with the CE-IVD kit. Non-invasive, about 2 minutes. The sample travels with a unique code without identifying data.
5. Laboratory analysis
ISO 15189/9001/17025-certified European laboratory. Process: DNA extraction, genotyping, quality control (98%+), ClinVar/GWAS/CPIC annotation.
If the sample doesn't meet quality threshold: second attempt, then free replacement kit if needed.
6. Results and communication
Results communicated exclusively to the data subject via authenticated digital report. Not communicated to third parties without explicit consent.
Each data point includes rs-ID, source database, and evidence level.
7. Test limitations
The test analyzes predefined SNPs, not the entire genome. Absence of risk variants does not exclude developing a condition. Associations may evolve. The test does not consider environmental factors and is not designed for prenatal diagnosis or rare mutations.
8. Implications for family members
Genetic data may reveal information relevant to biological family members. The data subject is invited to consider this.
Genomatik does not disclose results to family members without explicit consent.
9. Right not to know
The data subject has the right not to know the results, wholly or partially. Contact privacy@genomatik.com.
10. Retention and destruction
Sample: max 12 months by the laboratory, then destroyed.
Genetic data and report: 24 months. Early deletion on request.
Early destruction: request to privacy@genomatik.com. Irreversible.

12. Withdrawal of consent
Consent may be withdrawn at any time by contacting privacy@genomatik.com.
Consequences: cessation of processing, deletion of data and report (on request), sample destruction if still held.
13. Declaration of the data subject
By proceeding with the order and sending the biological sample, the data subject declares:
Having read and understood this Informed Consent and the Privacy Policy. Being of legal age. Providing explicit consent to genetic data processing. Being aware of the informational nature, limitations, and family implications of the test. Being aware of the right to withdraw consent.
